Cholesterol Screening:

  • Includes total cholesterol, triglycerides, HDL, and LDL.
  • Recommended every 5 years for everyone over the age of 20.

Blood Pressure Screening and Monitoring:

  • Recommended at least every 2 years for adults, and more often for those at higher risk for heart disease.

Diabetes Screening and Monitoring:

  • Fasting blood glucose, random blood glucose.
  • Screening recommended every 3 years for everyone over age 45, and those under 45 with risk factors for diabetes.

Body Fat Analysis:

  • For health-conscious individuals, athletes, or anyone interested in losing weight or monitoring their
    weight loss.

Features:

  • Results in less than 5 minutes.
  • Results can be sent to the primary care physician.

Fasting or Random Blood Glucose

Screening recommended every 3 years for everyone over age 45, and those under 45 with risk factors for diabetes.

Features:
Requires a fingerstick and small amount of blood.
Results within 5 minutes.

Additional Information:
Must be fasting in order to obtain an accurate fasting blood glucose reading 8-12 hours
(water, black coffee and medications are OK).

Hemoglobin A1c (HbA1c)

A1c recommended at least 2x/year for patients diagnosed with diabetes, and quarterly for patients not at goal, or have had a change in therapy.

Features:
Requires a fingerstick and small amount of blood.
Results within 5 minutes.

Additional Information:
Patient is not required to be fasting.

  • Comprehensive medication review to identify, prevent and resolve drug therapy problems.
  • Includes all Rx, OTC and herbal medications.
  • During the session, a pharmacist can identify ways to optimize the patient’s medication, as well as identify potential cost-savings opportunities.

Features

Face-to-face meeting with a pharmacist to include:
Monitoring of safety and efficacy of medication therapy.
Monitoring of patient’s response to medication therapy.
Education and training on appropriate use of medications.
Strategies to improve patient’s adherence to medication regimen.
Recommendations to patient and physician regarding selection, initiation or modification of medications.

  • This program is designed to help patients effectively quit smoking through a series of visits and follow-up phone calls with a trained pharmacist over a 3-month time period.
  • Patients will also receive guidance in selecting nicotine replacement products at Kroger, which are a proven benefit
    in smoking cessation.

Features

  • 12-week program combines in-store appointments and phone consultations for maximum benefit.
  • Program is tailored to meet the needs of each individual patient and can include multiple forms of nicotine replacement therapy or medication if prescribed by patient’s physician.
  • Patients meet one-on-one with a pharmacist trained in smoking cessation.
  • Pharmacist will help patient identify barriers to quitting & develop a strategy to overcome them.
  • This program aims to help patients safely lose weight and maintain weight loss.
  • Weight loss has been shown to help lower cholesterol, blood pressure and blood sugar through diet and exercise changes, without resorting to medications.

Features

  • A 12-week step-by-step program.
  • Trained pharmacist will perform a health assessment that includes screenings for diabetes, hypertension, and dyslipidemia.
  • Additionally, the pharmacist will identify the patient’s risk for weight-related illnesses, and will provide nutrition and exercise counseling.
  • A registered dietician will work with the patient to develop individual meal plans to help the patient meet their goals and schedule a grocery store tour.

Diabetes Education

  • Diabetes Self-Management Education (DSMN) is a multidisciplinary education program designed for patients with diabetes and their families that follows the American Diabetes Association's (ADA) recommended National Standards for Diabetes Self-Management Education (DSMN).
  • The goal of the DSME program is to provide quality education, training, and tools to patients with diabetes, enabling the patient to understand and contribute to the management of their own health.
  • DSME is a 10-Hour course that addresses: "Diabetes Basics", "Basic Nutrition", "Advanced Nutrition", and "Physical Activity and Controlling Complications".
  • The instructional team includes Registered Dieticians, Certified Diabetes Educators, and Doctor of Pharmacy Educators with certification in diabetes management.
  • The education program is available to any patient with diabetes, but may most benefit those who are newly diagnosed, poorly controlled, or are changing therapy modalities.

Continual Coaching

  • Diabetes Coaching takes the foundation of knowledge the patient receives during DSME and applies it to their everyday life through goal settings and behavior modification.
  • Patients are seen every 1 to 3 months as determined by their pharmacy coach.
  • Registered Dieticians are utilized for advanced carbohydrate counting and meal planning.
